Moving from Paediatric to Adult Care


 An invitation to our adolescent and parent members



Are you a teenage member of Heart Children Ireland? Are you a parent member of a child or adolescent? If so, we invite you to our upcoming Transition Information Day, all about moving from paediatric to adult cardiac care.


When:                Saturday, June 30th 2018 11.30am to 4pm


Where:               Carmichael Centre, North Brunswick Street, Dublin D07RHA8 (the same building where you find the Heart Children Ireland office).


Speakers:            We are delighted to let you know that the speakers on the day will include:



A person who has transitioned from paediatric to adult care in recent years.


A parent of a person who has transitioned from paediatric to adult care in recent years.


A representative from the Cardiac Team at Our Lady’s Children’s Hospital in Crumlin.


A representative from the Adult Congenital Heart Disease Unit at the Mater Hospital.


A Clinical Psychologist who is currently working with adolescent, adult and parent members.


A representative from Heart Children Ireland.


Refreshments:  Tea/coffee/scones will be provided during registration. Sandwiches will be provided at lunch time.


Parking:           Parking is available in the school opposite the centre, the entrance is almost directly opposite the entrance to the Carmichael Centre.
